Explanation of a Reiki Healing Session, Confidentiality, Disclaimers, Liability Waiver/Release and Your Understanding

Your Understanding

I understand that the Reiki Healing Session given involves a simple and natural hands-on method of energy balancing for the purpose of pain management, stress reduction, and relaxation. I understand that the practitioner will be placing hands on me during the Reiki Healing Session. I understand that while the Reiki practitioner does not diagnose conditions, nor do they prescribe medications, Reiki can complement any medical or psychological care I may be receiving. It is recommended that I seek a licensed health care professional for any physical or psychological ailments I may have. I also understand that long-term imbalances in the body sometimes require multiple sessions in order to facilitate the level of relaxation needed by the body to heal and balance itself.

Confidentiality

Any information that is discussed within the context of a Reiki/Distant Reiki/Home Smudging session is confidential between the Client and Practitioner. The Practitioner shall not disclose to a third party any information about a client, including the identity of the client, either during or after the lifetime of the client, without the consent of the client, client’s legal representative or the parent or guardian if the client is under 18, unless it is required by due process of the law.

The Reiki Chamber Disclaimers

1. I am NOT a medical doctor. My services or information on my website or at The Reiki Chamber location is not intended to be a substitute for medical advice. You should consult your physician in matters relating to your health conditions that may require diagnosis or medical attention.

2. The Reiki Chambers therapy table holds up to 400 lbs. Therefore, I only provide Distant Reiki to clients that surpass the weight limit.

3. Reactions and results may vary in each individual with every treatment.

4. I currently Do Not offer Reiki treatments to individuals with a PACEMAKER.

5. Payment due at time of session unless other arrangements have been made or you pay online when scheduling.

6. If you need to cancel or reschedule your session, please call/text me at (740) 318-1844 or thereikichamber@outlook.com at least 24 hours in advance.

7. There are NO Refunds on these services unless you have paid upfront and are cancelling your appointment within the allotted 24 hour time period.

Liability Waiver/Release

I hereby release and agree to hold The Reiki Chamber/Michelle Weber, harmless from and waive on behalf of myself, my heirs and any representatives of any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act or failure to act of The Reiki Chamber/Michelle Weber, or that may otherwise arise in any way in connection with any services received from The Reiki Chamber/Michelle Weber. I understand that this release discharges The Reiki Chamber/Michelle Weber from any liability or claim that I, my heirs or any representatives may have against The Reiki Chamber/Michelle Weber with respect to any bodily injury, illness, death, medical treatment or property damage that may arise from or in connection to any services received from The Reiki Chamber/Michelle Weber. I declare that the info I have provided is accurate and complete.
